Report Description Table of Contents Alpha-Emitter Radiopharmaceuticals Market Commercial Growth Depends on Pipeline Conversion Rather Than Patient Volume Alone The Global Alpha-Emitter Radiopharmaceuticals Market was valued at USD 240 million in 2025 and is projected to reach USD 4.10 billion by 2032, growing at a CAGR of 50.0%, according to Strategic Market Research. The forecast represents an absolute revenue increase of USD 3.86 billion. It assumes that alpha-emitter therapies will move from a market centred on one established radium-223 product toward a broader commercial portfolio built around actinium-225 and lead-212. Alpha-emitter radiopharmaceuticals deliver radioactive isotopes to selected tumour targets. Their commercial potential comes from treating defined cancer populations that may have exhausted established therapies. The market includes finished medicines, pharmaceutical-grade isotopes, clinical manufacturing, regional dose production and associated distribution services. It should not be measured only through the number of drug candidates because most pipeline programs have not yet produced commercial treatment revenue. The 2025 revenue base remains concentrated around Bayer’s Xofigo. The radium-223 therapy is approved in more than 50 countries for metastatic castration-resistant prostate cancer with symptomatic bone metastases and no known visceral metastatic disease. It also has a defined U.S. billing route under HCPCS code A9606. This gives the market a proven reimbursement and hospital-procurement structure, but the approved population is narrow. Claims depend on diagnosis, disease location and supporting documentation rather than broad prostate cancer incidence. Bayer strengthened the commercial outlook for radium-223 in February 2026 when the Phase III PEACE-3 trial reported a 24% reduction in the risk of death for radium-223 plus enzalutamide compared with enzalutamide alone. Median overall survival reached 38.2 months versus 32.6 months. A successful regulatory expansion could increase radium-223 use earlier in the treatment pathway and extend the commercial life of Xofigo. The result does not represent additional approved revenue until regulators authorize the combination. The larger forecast opportunity comes from therapies that are still clinical or preclinical. Commercial growth therefore depends on four conversions: trial results into approvals, isotope capacity into qualified pharmaceutical supply, manufacturing sites into released treatment doses and eligible patients into reimbursed procedures. Failure at any one stage would slow the movement from pipeline valuation to market revenue. Revenue Is Expected to Shift Toward Actinium-225 and Lead-212 Strategic Market Research estimates that radium-223 accounted for approximately 62% of 2025 revenue, equal to USD 148.8 million. Actinium-225 represented an estimated 23%, or USD 55.2 million. Lead-212 contributed about 10%, or USD 24.0 million, while thorium-227, astatine-211 and other alpha emitters accounted for the remaining 5%, or USD 12.0 million. These figures are analytical allocations based on commercial approvals, development-stage procurement, manufacturing activity and disclosed isotope agreements. They are not reported manufacturer revenue shares. By 2032, actinium-225 is projected to generate approximately 49% of the market, equivalent to USD 2.01 billion. Lead-212 is expected to reach 24%, or USD 984 million. Radium-223 would represent about 21%, or USD 861 million, despite continuing to grow in absolute value. Other alpha emitters are projected to contribute 6%, or USD 246 million. Actinium-225 has the largest forecast opportunity because it supports several targeting approaches across prostate cancer, neuroendocrine tumours, breast cancer, lung cancer and blood cancers. Its expansion is not yet limited by developer interest. It is limited by the amount of qualified isotope that companies can secure for trials and future launches. A supplier estimate recorded by the European Commission’s Joint Research Centre placed worldwide actinium-225 supply at approximately 2.5 curies annually. The same presentation indicated that 500–800 curies could eventually be required to support major development programs. The estimate is not an audited demand forecast, but it shows why access to actinium-225 has become a transaction, procurement and competitive issue rather than a routine raw-material purchase. The U.S. Department of Energy increased its largest disclosed actinium-225 batch to 112 millicuries after addressing a previous 50-millicurie processing constraint. TerraPower began weekly commercial-scale production in October 2024 and stated that its material was being used in human trials conducted by several drug developers. These developments reduce dependence on small research batches. They also allow developers to plan larger studies, qualify alternative suppliers and negotiate multi-year contracts. Actineer added another supply-chain signal in July 2026 by reporting a closed-loop process for recycling radium-226, a precursor used in actinium-225 production. Recycling does not remove every manufacturing constraint, but it can improve precursor utilization and reduce exposure to scarce inventories. This matters commercially because production continuity will influence which drug developers can complete trials and launch in several countries without rationing doses. Lead-212 has a smaller pipeline than actinium-225 but a more visible industrial model. Orano Med disclosed combined capacity for 10,000 doses annually from 2025 through its French and U.S. sites. It intends to increase that capacity tenfold by the end of the decade. Because lead-212 has a 10.6-hour half-life, treatments need to be manufactured near hospitals. This favours companies capable of financing several regional production sites rather than relying on one centralized plant. By application, prostate cancer is estimated to have represented 66% of 2025 revenue, or USD 158.4 million. Neuroendocrine tumours accounted for approximately 16%, or USD 38.4 million. Haematological malignancies represented 8%, or USD 19.2 million, while other solid tumours contributed 10%, or USD 24.0 million. Prostate cancer is projected to retain the largest application position in 2032, but its share is expected to decline to 45% as other indications produce commercial revenue. Neuroendocrine tumours are projected to reach 24%. Haematological malignancies are expected to contribute 12%, while breast, lung and other solid tumours collectively account for 19%. The United States is expected to record 333,830 new prostate cancer cases in 2026, with 9% diagnosed at the distant stage. This defines the underlying disease burden but not the alpha-therapy market. Xofigo eligibility is substantially narrower, while actinium-based prostate treatments may require PSMA expression, prior therapy and other clinical criteria. Patient incidence therefore supports the development opportunity but should not be converted directly into revenue. Competition Is Being Repriced Around Integrated Platforms Bayer remains the commercial incumbent because Xofigo combines regulatory approval, international market access and established payer procedures. PEACE-3 could defend that position by supporting use of radium-223 in combination with an androgen-receptor inhibitor. Bayer’s advantage is not isotope novelty. It is a functioning treatment, reimbursement and hospital-distribution model that newer entrants must still build. Bristol Myers Squibb paid approximately USD 4.1 billion for RayzeBio and subsequently opened a 77,000-square-foot Indianapolis facility for actinium-based radiopharmaceuticals. The facility was operational for clinical orders in 2025 and was designed to support tens of thousands of annual doses at capacity. The acquisition reduced BMS’s dependence on external manufacturing and gave it control over development, production and dose release. RayzeBio’s RYZ101 is an actinium-225-labelled therapy targeting somatostatin receptor 2. It is being evaluated in a global Phase III study for patients with gastroenteropancreatic neuroendocrine tumours that progressed after lutetium-177 therapy. This positioning is commercially important because it targets an identifiable post-radioligand population with limited approved options. It also creates a pathway through which alpha therapy could follow rather than replace beta-emitter treatment. AstraZeneca’s approximately USD 2.4 billion acquisition of Fusion Pharmaceuticals added actinium-based programs and production capabilities. The transaction shows that large pharmaceutical companies are buying manufacturing preparedness together with clinical assets. An effective oncology candidate has limited value if isotope shortages prevent trial expansion or commercial supply. Novartis paid approximately USD 1 billion upfront for Mariana Oncology, with up to USD 750 million in potential milestones. Mariana was a preclinical-stage company. Its valuation therefore reflects the expected value of an alpha-emitter research platform, specialist personnel and clinical-supply infrastructure rather than existing product sales. The transaction also allows Novartis to extend its established radioligand business beyond lutetium-177 into earlier-stage alpha programs. Sanofi entered the market through both product rights and infrastructure investment. It secured global commercialization rights to AlphaMedix and committed EUR 300 million for an approximately 16% interest in a lead-212-focused Orano Med entity valued at EUR 1.9 billion. This structure gives Sanofi access to a clinical asset and its isotope platform without purchasing the whole company. AlphaMedix provides one of the strongest clinical signals in the lead-212 segment. Among 35 radioligand-therapy-naïve patients in Phase II, the blinded independent central review objective response rate was 57.1%. Among 26 previously treated patients, the corresponding rate was 19.2%. The results suggest stronger commercial positioning in treatment-naïve disease than in the salvage setting. The FDA granted Breakthrough Therapy Designation for eligible PRRT-naïve GEP-NET patients, and an international Phase III study is being planned. AlphaMedix remains investigational. Perspective Therapeutics and Lantheus illustrate a different competitive model. Lantheus paid USD 28 million for licensing and co-development options and agreed to invest up to approximately USD 33 million in Perspective. The arrangement also included the transfer of a New Jersey manufacturing facility. Smaller developers can therefore monetize pipeline optionality while obtaining capital and manufacturing assets before pivotal-stage validation. North America Leads Market Access While Europe Builds Supply Depth Strategic Market Research estimates that North America accounted for 49% of 2025 revenue, equal to USD 117.6 million. Europe represented 32%, or USD 76.8 million. Asia-Pacific contributed approximately 14%, or USD 33.6 million, while Latin America and the Middle East and Africa collectively accounted for 5%, or USD 12.0 million. North America’s position is supported by the complete commercial chain rather than cancer incidence alone. The United States has a reimbursed radium-223 treatment, DOE isotope production, TerraPower’s weekly actinium-225 output and large manufacturing investments from BMS and Orano Med. Canada adds cyclotron production and precursor-processing capabilities through Canadian Nuclear Laboratories and Actineer. Europe’s advantage lies in isotope expertise, public funding and distributed manufacturing. Orano Med’s French lead-212 facility represents EUR 29 million of investment and received almost EUR 3.8 million in public support. The company is also developing an industrial facility dedicated to thorium-228, the precursor used for lead-212 production. France is therefore supporting both the upstream isotope chain and finished-dose manufacturing. By 2032, North America is projected to represent 44% of market revenue, or USD 1.80 billion. Europe is expected to account for 31%, or USD 1.27 billion. Asia-Pacific is projected to increase to 20%, or USD 820 million, as regulatory access and nuclear-medicine capacity expand. The remaining regions are expected to contribute 5%, or USD 205 million. Asia-Pacific’s higher forecast share depends on local approvals, reimbursement and qualified treatment sites. Japan projected 99,900 new prostate cancer cases in 2025, but public evidence does not identify the number eligible for alpha-emitter therapy. The region’s commercial opportunity will therefore be determined by treatment infrastructure and product access rather than the broad cancer population alone. Isotope Availability Will Influence Pricing and Launch Sequence The alpha-emitter radiopharmaceutical market does not yet operate through standardized isotope spot prices. Developers secure material through preferred-supplier contracts, predetermined quantities and volume-based terms. ITM’s 2025 actinium-225 agreement with Alpha-9 Oncology demonstrates that supply is being contracted while products are still under development. Early agreements protect trial continuity, but they can also tie developers to suppliers whose scale-up schedules remain unproven. Scarcity gives reliable isotope producers pricing power, although larger customer commitments may secure volume discounts. The more important commercial benefit is launch certainty. A company with several qualified isotope sources can enrol trials faster, add countries and support treatment demand after approval. A developer with one constrained supplier may need to limit site activation or delay geographic expansion. Manufacturing location will also affect market share. Short-lived products cannot be stocked and distributed like conventional oncology medicines. Regional facilities must produce, test and release doses against scheduled hospital appointments. Companies that control production near major treatment centres can reduce delivery risk and serve more patients from the same clinical asset. Reimbursement creates the final commercial threshold. Xofigo shows that product approval must be followed by coding, indication-specific documentation and treatment-site readiness. Future actinium-225 and lead-212 products will need payment structures that cover the drug, dose preparation, administration and nuclear-medicine services. Products with strong clinical results may still experience slow adoption if hospital economics are unattractive. The projected USD 4.10 billion market therefore depends on more than scientific progress. It requires multiple approved products, reliable isotope output, regional manufacturing and reimbursement that allows hospitals to treat patients without absorbing the cost. Companies that control these elements are positioned to convert pipeline value into recurring revenue. Companies without supply and market-access infrastructure will remain dependent on licensing partners, acquisition buyers or external manufacturers. 7.1.
